I managed to get that wierd glitch again (the character dissapeared, and is probably somewehere above the level). It happened when the character jumped and got pressed against the wall, but was a bit to high to be squished and a bit too low to be on top of the block.

[edit] OMG! It happened again! It definitely has something to do with the character being to high to be squashed but too low to be on top of the block.
